We are excited about your decision to allow Lifespan Family Healthcare to become involved in your health and well-being. Here are some important things to remember about the office and links to the forms you will need to become a new patient.

(Please note: Our office phone service is available at 8:15 am to 1:00 pm
and from 2:00 pm to 4:30 pm daily.)

Payments & Insurance Billing:
As a courtesy to our patients we will submit insurances claims. Please be aware that some or perhaps all of the services rendered may or may not be covered. If your insurance company denies payment, you will be billed.
Please bring your insurance card(s) for each visit. Please be aware it is your responsibility to know your co-pay and/or deductible information, along with information on covered services. If your insurance cannot be verified, payment in full is due at the time of service. Once your insurance has been verified, we require payment of co-payments as defined by your primary carrier. We accept cash, checks and major credit cards.
A minimum of $35.00 fee will be assessed on all returned checks. If you do not have medical insurance, payment is due at time of service. If you need special arrangements, please contact our office manager, prior to your appointment.
We participate with most major insurance plans, including those listed below. Please remember you are responsible for any bill or portion of a bill that is not paid by your insurance company. If we do not participate with your plan, you are responsible for the full amount not paid by the insurance plan.

Minors need to be accompanied by a parent, especially on the first visit
Appointment Cancellations/No show policy (IMPORTANT PLEASE READ) :
Please give 24 hours notice if an appointment needs to be re-scheduled or canceled. Not showing up for an appointment hinders our ability to care for you as well as others, because of this, three (3) “no-shows” could be grounds for dismissal from the practice. We charge a no-show fee of $35 payable prior to or on your next appointment.
